Your HSA is an FDIC insured bank account. In compliance with Section 326 of the USA PATRIOT Act, we are required to obtain, verify, and record information that identifies each person who opens an account.

When you open a Vita HSA account, we will ask for your name, address, date of birth, and social security number to allow us to identify you. Your information will be verified using a database maintained by a third party. If your identity cannot be verified, you will be required to provide documentation that verifies your identity.

If additional documentation is required, you will receive an email notifying you of what information could not be verified, and you will be instructed to provide a document that validates your information.
