In general, your employee benefits are not protected while on a leave except under FMLA or USERRA. 

Personal Leave Policy

Some employers do provide an internal policy that allows for continuation of some benefits in in certain circumstances, if you have a personal leave of absence that is approved by Employer. Benefits may only continue to the extent that Employer has a standing corporate policy that outlines such continuation, that policy is in writing, and that policy is approved in advance by the insurance carrier. Otherwise, benefits will not be continued in the event of a non-USERRA, non-FMLA leave of absence.
